Service Description
Management of communicable and non communicable diseases-hypertension, diabetes, copd, asthma, metabolic syndrome, uncomplicated thyroid conditions and early identification and referral for rheumatological conditions. Living with a long-term condition means managing it every day — not just at appointments. My role is to make that easier. I look after patients with non-communicable conditions including high blood pressure, diabetes, raised cholesterol, asthma and thyroid disorders, as well as communicable conditions such as HIV and tuberculosis. My diploma in HIV Management means I can offer informed, non-judgemental care in an area where consistency and confidentiality matter enormously. Care usually begins with a full consultation: your history, your current medication, what's working and what isn't. From there we agree a plan — adjusting treatment where needed, arranging blood tests or investigations through a laboratory near you, and setting a schedule for reviews so nothing drifts. Because consultations happen by video, keeping up with reviews doesn't mean taking time off work or travelling. That consistency is often what makes the difference in long-term conditions. Some things do need hands-on assessment or in-person testing, and I'll always tell you clearly when that's the case and help arrange it locally.
